How treatment is selected: Most acute sinus infections are viral and improve with hydration, saline rinses, pain relief, and time. Bacterial sinusitis becomes more likely when severe symptoms persist, symptoms last more than about ten days without improvement, or illness improves and then worsens. In those cases, a clinician may prescribe an appropriate antibiotic based on allergies, pregnancy, recent antibiotics, and local guidance.
